  • Birth Certificate - please review the requirements and acceptable documents needed here

  • Genealogy - please call for pricing.

  • Instructions for obtaining a vital record are on the form. Either form you choose below WILL need to be printed, signed and mailed to us. Be sure to fill out all fields in the section for the document you are requesting.

  • When submitting your form be sure to include a self-addressed stamped envelope, a copy of a signature ID with photo and a check payable to McDonough County Clerk for the amount due. This form CANNOT be submitted online, it MUST be mailed.
